Heparin is an anticoagulant (blood thinner). The 25,000 IU strength is typically supplied in multidose vials used in hospital or clinical settings.
Heparin 25,000 IU is used for:
Prevention and treatment of deep vein thrombosis (DVT)
Prevention and treatment of pulmonary embolism (PE)
Prevention of clot formation during and after surgeries
Prevention of clotting in dialysis, cardiac bypass, and blood transfusion procedures
Management of unstable angina and acute coronary syndromes
Treatment of certain blood clotting disorders
Exact dosing is always based on a doctor’s decision, monitoring, and patient condition.
Initial IV bolus: Commonly 5,000 units
Maintenance IV infusion: Adjusted based on aPTT results, e.g.,
1,000–1,500 units per hour, titrated to maintain therapeutic clotting levels
Subcutaneous prophylactic dose: Usually 5,000 units every 8–12 hours
Dosage varies depending on body weight, indication, and lab monitoring.
Common:
Bleeding (nose, gums, bruising)
Local irritation or pain at the injection site
Less common:
Thrombocytopenia (low platelet count)
Elevated liver enzymes
Osteoporosis with long-term use
Administered intravenously (IV) or subcutaneously (SC)
Should be used under hospital supervision because dose adjustments require regular aPTT monitoring
Never inject intramuscularly
Avoid mixing with other medications in the same syringe unless instructed
