Medical Uses

Tofacitinib is an oral Janus kinase (JAK) inhibitor used to treat autoimmune inflammatory conditions. It helps reduce inflammation by lowering overactive immune system activity.

It is commonly prescribed for:

• Rheumatoid Arthritis (RA)

For adults with moderate to severe RA who have not responded well to methotrexate.

• Psoriatic Arthritis (PsA)

Used when conventional treatments are not adequate.

• Ulcerative Colitis (UC)

For moderate to severe active ulcerative colitis.

• Juvenile Idiopathic Arthritis (JIA)

In children (specialized dosing; doctor determines).

Dosage (Doctor Must Decide)

Typical adult dosing guidelines:

Rheumatoid Arthritis / Psoriatic Arthritis

• 5 mg twice daily

Ulcerative Colitis

• Induction: 10 mg twice daily (for a limited duration)

• Maintenance: 5 mg or 10 mg twice daily depending on response

Notes:

• Dosage may change based on kidney or liver function

• Dose reduction may be required when used with interacting medications

Side Effects

Common:

• Headache

• High cholesterol levels

• Nasal congestion or sore throat

• Diarrhea

• Mild increase in liver enzymes

Serious:

• Increased risk of infections (e.g., TB, shingles)

• Blood clots (especially at higher doses)

• Heart-related events (particularly in high-risk patients)

• Low blood counts

• Gastrointestinal perforation (rare)

Regular monitoring is required.

How to Use

• Taken orally, with or without food

• Take at the same time every day

• Do not miss doses; if missed, take as soon as remembered unless close to next dose

• Doctors perform regular tests to monitor:

  • CBC (blood counts)

  • Liver function

  • Lipid levels

  • Infection screening (TB, hepatitis)

Avoid use if signs of active infection are present.
