Tofacitinib is an oral Janus kinase (JAK) inhibitor used to treat autoimmune inflammatory conditions. It helps reduce inflammation by lowering overactive immune system activity.
It is commonly prescribed for:
For adults with moderate to severe RA who have not responded well to methotrexate.
Used when conventional treatments are not adequate.
For moderate to severe active ulcerative colitis.
In children (specialized dosing; doctor determines).
Typical adult dosing guidelines:
5 mg twice daily
Induction: 10 mg twice daily (for a limited duration)
Maintenance: 5 mg or 10 mg twice daily depending on response
Dosage may change based on kidney or liver function
Dose reduction may be required when used with interacting medications
Common:
Headache
High cholesterol levels
Nasal congestion or sore throat
Diarrhea
Mild increase in liver enzymes
Serious:
Increased risk of infections (e.g., TB, shingles)
Blood clots (especially at higher doses)
Heart-related events (particularly in high-risk patients)
Low blood counts
Gastrointestinal perforation (rare)
Regular monitoring is required.
Taken orally, with or without food
Take at the same time every day
Do not miss doses; if missed, take as soon as remembered unless close to next dose
